Initial Situation
Patient presented with failing front teeth with old gold restorations in 2003. Two teeth were removed and a all ceramic bridge fitted. Unfortunately the two supporting teeth of the bridge failed with root fractures and abscess. The decision was made to remove the two pillars of the front bridge and plan immediate Z5M(t) Z-Systems all zirconia dental implants. This would involve removal of the two supporting teeth and immediate placement of implants in the sockets as well as place two other implants in the ridge that has not had teeth since 2003. This was planned using a Cone beam CT image and planning software shown.
Info about the treatment
In one surgical visit blood was drawn to be used to produce an injectable platelet rich fibrin and also an more solid gelatinous form platelet rich fibrin. This provided a material containing all the bodies healing qualities to be used instead of any grafts derived from pig, cow or other human. This was used to create “sticky bone” that filled the spaces that were needed to support the process. All four implants were placed after surgical removal of the two remaining dead teeth. A surgical soft tissue flap was raised and after placing supportive blood products was closed with sutures that did not put any pressure on the surgical site. A full arch temporary bridge was fabricated . To support this bridge the old crowns of the teeth either side of the site was removed and incorporated into the 6 unit temporary structure. 6 months later the temporary bridge was removed and implants prepared for final long term individual restorations. No injections were needed for this. All Zirconia crowns beautifully crafted by Werner Sauer Smile Design dental laboratory were fitted with no injections. All ready for the 70th birthday party. A very happy patient.
